The film is actually called Hell Behind Bars from 1984. It's kind of rare to see anal torture like this as I guess it's not well received in polite society but in reality the anus has almost certainly been used for torture for centuries. I guess there aren't many who wouldn't confess to just about anything with a sharp spike stuck up their asshole!
